I'm trying to work out how to blog for future reference. The idea is to promote some of my experiences. I think my dentistry is a pretty good average. I never entirely made the full jump after over twenty years go from a busy NHS GDP to spending twice as long and charging accordingly. The photos were never meant to be seen as I'm not an academic. The Blog is to help promote and improve dentistry. It's too late for me to improve my digital imaging, but not too late for you to think and discuss our ideas from everyday GDP experience, ideally at peer review. We need all kinds of support. This is from memory…… Mrs * didn't like me, nothing personal. She had a bad experience leaving her with a poorly fitting twenty-year old p/- she liked. Despite my best efforts, she was anxious and possibly didn't trust me. One day over six years ago her UL1 crown fractured off along with the core, and I replaced it with a Cerec in an hour much to her surprise, and mine. In January 06 she presented with the ur3 crown fractured off. As it was an abutment to the denture, I suggested an overdenture. It was cheaper and in my view the best option. For someone who hated us, I was surprised when she wanted me to attempt a crown with no guarantee, as "it worked last time!". After a minimal prep, simple a shoulder I fitted another crown, using my blue cam cerec, I made the crown. And of course, it was one visit as she couldn't go without the denture for the day. A few weeks later she rang the reception as she wasn't happy, and made an appointment to see me. Had it come off? It fitted well, not brilliant aesthetics, but a good match for her denture. No, she liked the look of her teeth, would I crown the remaining ones? I told her I wasn't keen, and I don't like removing anterior crowns as sometimes you find it's only the decay holding them in place as with the UL1. I would get the blame for ultimately an extraction of what was apparently a sound tooth. And once a no win no fee lawyer rang her……. and as she rarely smiled no one could see her crown margins.😏 As of this week, August 2020 she is still happy with her appearance, and her old P/- which fits tighter now as the pal margins are ideal as the scan was taken with her denture in situ.
However, when making this number of crowns taking an impression and two visits would have been better time-wise. I think this was all on the NHS, but I'll have to check. |
